Job Title: Project Lead / Relationship Manager
Location: Essex and Ipswich (Hybrid)
Hours: 37.5 hours per week (flexibility required)

Job Purpose

This job description lists the main areas in which you will be expected to provide support. We may need to add more specific tasks to this based on the needs and wishes of the people we support, our workforce and the needs of the organisation.

To lead the development of new and existing supported living placements for individuals with complex needs, by fostering strong partnerships with commissioners, social care professionals, and health service stakeholders. The role will ensure timely response to referrals, effective assessment coordination, and appropriate placement matching, including negotiation of funding packages. This is a strategic and operational role pivotal to the organisation’s growth and reputation for excellence in specialist supported living.

Main Duties and Responsibilities

Relationship Development & Stakeholder Engagement

·         Build and maintain strong, strategic relationships with local authority and NHS commissioners, case managers, brokerage teams, social workers, care coordinators, and multidisciplinary professionals.

·         Promote the service through networking, attending panels, forums, and relevant sector events.

·         Act as the main point of contact for key external partners, ensuring excellent communication and responsiveness.

Referral & Placement Coordination

·         Manage and track incoming referrals, ensuring prompt triaging, assessment coordination, and follow-up.

·         Lead on sourcing and matching appropriate supported living placements for individuals based on their clinical, behavioural, and social needs.

·         Oversee person-centred assessments in collaboration with clinical/assessment teams.

·         Ensure comprehensive assessment reports and service proposals are prepared and submitted in a timely fashion by liaising with the Registered and Operational manager.

Pricing and contracting

·         Lead on the negotiation of placement costs and individual funding arrangements with commissioners and stakeholders.

·         Ensure clear costings reflect complexity of need, staffing requirements  and compliance with internal financial frameworks.

·         Work with finance and the operations manager to ensure contracts are managed effectively and reviewed regularly.

Project and Service Development

·         Identify and lead on new placement or service opportunities in line with our organisational strategy and market demand.

·         Contribute to the design of new supported living services to meet emerging needs and best practice standards.

·         Support the mobilisation of new packages, ensuring seamless handover to operational teams.

·         Lead on any other key projects that benefit the organisation.

Reporting and Governance

·         Maintain accurate records of referral and placement activity, providing regular reports to the senior leadership team.

·         Ensure compliance with regulatory standards and organisational policies.

·         Monitor outcomes of placements to inform quality assurance and service improvement.

Other:

·         Promote a company culture conducive to high-standard care and support delivery.

·         Ensure all information is kept up to date in accordance with GDPR legislation.

·         Develop key relationships with the Care Quality Commission, Local Authority, Health professionals and other key stakeholders.

·         Create an open and transparent culture enabling staff to confidently raise concerns and ensure there is clear evidence of lessons learnt when there have been incidents and near miss events.

·         Carry out any additional tasks or duties according to your Project lead/Relationship Manager responsibilities and within your scope of competence.

·         Act as an ambassador for Harley house supported living ensuring you always maintain professionalism and integrity.

·         Ensure you refrain from befriending staff or services users on social network sites

·         Always act in the best interest of any person who may lack capacity.

·         Ensure that the safeguarding of the people we support is always a priority.

·         Be aware of, and follow the procedure for, reporting any safeguarding concerns or incidents

·         Attend training and ensure all mandatory training is up to date.

·         Co-operate and attend a yearly appraisal with the Managing Director.

·         Ensure the health, safety, and welfare of the people we support and the workforce.

·         Embrace team working by ensuring effective and open communication for the benefit of the people we support

·         RNLD, RMN or Social worker

Experience

·         Proven experience in a relationship management or commissioning-facing role

·         Experience working with individuals with complex needs (LD, autism, MH, behaviours that challenge)

·         Experience managing referrals and assessment processes

·         Previous registered manager or leader of a regulated activity

·         Experience of negotiating care packages with commissioners or public bodies

·         Experience mobilising new supported living services or specialist placements

Knowledge

·         Knowledge of commissioning pathways and funding mechanisms (e.g. CHC, Section 117)

·         Familiarity with complex behavioural needs and trauma-informed approaches

·         Knowledge of Positive Behaviour Support (PBS), person-centred planning

·         Clinical knowledge associated with physical disabilities

Skills and Attributes

·         Excellent interpersonal, negotiation and communication skills

·         Project management and organisational skills

·         Strong report writing and presentation abilities

·         Commercial awareness and cost modelling
